Layer Referencing Bug

I don't know why this keeps happening but it has us to our wits end. We have layer referencing files and every once in a while, the files go all bugeyed. Walls, doors and other symbols "move" to places that they aren't supposed to be. The only way to fix this is to enter the source file, (which displays the correct information), do something mundane and then save and update the reference in the target file.

As a workflow for a multi-user environment, this is cumbersome and extremely time consuming. If the task of having to update references and then viewports wasn't tedious enough already, having to deal with this is becoming extremely detrimental to our efficiency.

Here is a screen shot of a very basic instance of what is happening. In this instance, its just one door that moved. But in others walls will continue on for infinite or completely change location.
