Callout out tool questions

I don't understand where VW expects me to put the callouts on a sheet layer.

Are they supposed to go inside the viewport annotations? If so then why are the controls for it scaled to the viewport scale (rounding and shadowing are scaled).

If they are not, it becomes incredibly hard to work with them, as I can't select them cleanly (the viewport keeps selecting).

I wish I could bold some parts of a callout - like the callout title.

Having only a single text style available it a bit tough.
